Once-weekly subcutaneous administration of GLP-1R/GIPR dual agonist tirzepatide (up to 15 mg/week) significantly reduces HbA1c and body weight in patients with type 2 diabetes, with efficacy exceeding current GLP-1RAs.
If you have type 2 diabetes and struggle with weight, ask your doctor about tirzepatide. It is a once-weekly injection that has been shown to lower blood sugar and reduce body weight more effectively than older GLP-1 drugs. While injections can be a barrier, oral options exist for other drugs in this class.
GoodSupportsHIGH confidence
In studies over 40–52 weeks in individuals with T2D, the highest dose (15 mg/week) reduced HbA1c (by 2.5%; 28 mmol/mol) and body weight (by 12.9 kg) [23]. These effects are greater than generally observed with current GLP-1RAs suggesting that the GIP receptor agonism provides extra efficacy.
Why this rating
Based on clinical trials (40-52 weeks) in T2D patients, though the paper is a review summarizing these trials.

- Combining the amylin analogue cagrilintide (4.5 mg/week) with semaglutide (2.4 mg/week) results in greater weight loss (15.4% reduction) than either agent alone in obese or overweight individuals.Good
- Oral semaglutide (7-14 mg/day) provides glucose-lowering and weight-loss effects comparable to subcutaneous formulations, though with lower bioavailability requiring higher doses.Good

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
