Combining the amylin analogue cagrilintide (4.5 mg/week) with semaglutide (2.4 mg/week) results in greater weight loss (15.4% reduction) than either agent alone in obese or overweight individuals.
For significant weight loss, combining cagrilintide and semaglutide has shown a 15.4% body weight reduction in 20 weeks. This combination therapy is an option for those who do not achieve sufficient weight loss with single-agent GLP-1 therapy.
GoodSupportsHIGH confidence
When obese or overweight people received cagrilintide (4.5 mg/week) in combination with semaglutide (2.4 mg/week) there was a 15.4% reduction in body weight at 20 weeks [31]
Why this rating

- Once-weekly subcutaneous administration of GLP-1R/GIPR dual agonist tirzepatide (up to 15 mg/week) significantly reduces HbA1c and body weight in patients with type 2 diabetes, with efficacy exceeding current GLP-1RAs.Good
- Oral semaglutide (7-14 mg/day) provides glucose-lowering and weight-loss effects comparable to subcutaneous formulations, though with lower bioavailability requiring higher doses.Good

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
