In non-beta tissues (liver, kidney, skeletal muscle, adipose), the mitochondrial benefits of GLP-1RAs are likely a combination of direct receptor signaling and indirect systemic improvements such as weight loss and reduced inflammation.
While GLP-1 drugs improve mitochondrial health in organs like the liver and muscles, these benefits are partly due to the overall reduction in body weight and blood sugar. It is a combined effect of losing weight and the drug's direct action on cells.
ModerateQualifiesMEDIUM confidence
In liver, kidney, and skeletal muscle, mitochondrial advantages are likely real but are more difficult to separate from secondary improvements in glycemia, adiposity, and systemic inflammation... Whether these are direct hepatocyte actions or mostly systemic effects is still unresolved.
Why this rating
Evidence is mixed and difficult to disentangle in human studies compared to beta cells.

- GLP-1 receptor agonists exert tissue-protective effects across multiple organs (pancreas, heart, liver, adipose, muscle, kidney, brain) primarily by driving mitochondrial remodeling, including improved biogenesis, quality control, and reduced oxidative stress.Good
- In pancreatic beta cells, GLP-1 receptor agonists directly protect against apoptosis and improve insulin secretion by enhancing mitochondrial ATP production, reducing oxidative stress, and promoting autophagy.Good
