Calories trump macro ratios
When total caloric intake and adherence are held equal, low-carb and low-fat diets produce statistically identical weight loss, and a directly controlled low-calorie group outperformed a low-fat group by roughly 4 to 7 kg across both sexes, proving that cutting a macro without controlling total intake is what usually fails people.

Food quality beats food type
Over 60% of the global diet-attributable type 2 diabetes burden comes from eating too much of specific harmful foods, namely refined grains, processed meats, and sugary drinks, not from eating carbs or fat as broad categories, so swapping refined for whole foods matters far more than eliminating a macro.

Fat type, not fat itself
Saturated fat from modern domesticated animal products and dairy raises LDL cholesterol and is linked to endotoxemia and inflammation, while replacing even 5% of daily calories from carbs with polyunsaturated fat meaningfully lowers HbA1c, fasting insulin, and HOMA-IR, meaning the type of fat you choose is the relevant variable, not fat as a whole.

Cutting fat can backfire neurologically
Selectively reducing dietary fat in people with obesity decreases dopamine D2/3 receptor binding in the striatum and blunts neural responses to food cues, which drives higher consumption of hyperpalatable foods combining fat and carbohydrate together, so demonizing and eliminating one macro can create a compensatory overconsumption spiral.
There is a real grain of truth here: specific high-quality swaps do matter. Replacing saturated fat with unsaturated fat genuinely improves cholesterol and metabolic markers, and certain foods like processed meats and refined grains are legitimately harmful in excess. The mistake is scaling that specific insight up to an entire macronutrient category.
Stop labeling a whole macro as the enemy and instead focus on replacing the specific low-quality versions of that macro, refined grains, processed meats, saturated-heavy dairy, with whole-food alternatives, while keeping total calories in check.
