Chronic energy imbalance (caloric excess) drives adipocyte hypertrophy and hyperplasia, leading to endoplasmic reticulum (ER) and mitochondrial stress, which causes systemic insulin resistance and inflammation.
Focus on correcting the root cause: energy imbalance. Reducing caloric intake and increasing energy expenditure reverses the cellular stress (ER and mitochondrial) that drives obesity-related disease. Lifestyle modification is the most promising intervention.

Chronic energy imbalance produces adipocyte hypertrophy and hyperplasia, endoplasmic reticulum stress, and mitochondrial dysfunction.
