Roux-en-Y gastric bypass (RYGB) surgery induces rapid, starvation-like adaptations in gut microbiota composition, specifically increasing Bacteroides/Prevotella and E. coli while decreasing Lactobacillus and Bifidobacterium, which correlates with reduced body fat and leptin levels independently of caloric intake for some species.

Bariatric surgery like RYGB triggers a rapid reorganization of gut bacteria that mirrors a starvation response, helping to reduce body fat and inflammation. This happens partly because the surgery changes gut pH and bile flow, not just because you eat less. For obese individuals, this microbial shift is a key part of why metabolic health improves so quickly after surgery.
