Berberine's effect on blood clotting is complex. While it generally inhibits platelet aggregation, it may increase tissue factor expression in some cell types, potentially promoting thrombosis. This highlights the need for caution in individuals with clotting disorders or those taking anticoagulants.

However, another study revealed that BBR could increase TNFα-stimulated TF expression in HAECs, and could decrease the expression of TF pathway inhibitor in HAECs and in arteries of apolipoprotein E-deficient mice (ApoE-/-) mice, what might promote thrombosis [104]. BBR therefore might act bidirectionally on TF expression in different cell types [105].
