CNS insulin action is dependent on intact leptin signaling, and leptin's effects on energy expenditure are mediated through melanocortin receptors (specifically MC4R).
Treating obesity often requires addressing the brain's downstream response (melanocortin system), not just the upstream hormones. If your body is resistant to leptin or insulin, targeting the final common pathway (like MC4R) might be more effective.
GoodQualifiesHIGH confidence
Intact leptin signaling appears to be required for insulin to exert its central effects... Furthermore, leptin’s effects on the thyroid axis also involve hypothalamic signaling via MC4R.
Why this rating
Supported by knockout mouse studies and receptor binding assays.

- Leptin and insulin act as redundant but complementary adiposity signals to the central nervous system (CNS), specifically the ventral hypothalamus, to regulate long-term body fat stores by modulating neuropeptide expression (NPY/AgRP and POMC/CART).Good
- Short-term meal satiety signals (like CCK) are modulated by the strength of long-term adiposity signals (insulin and leptin); higher adiposity signals increase sensitivity to satiety cues, reducing meal size.Good
